• Valid driver’s license or government-issued photo identification
• Glasses or contact lenses if you use them while driving
• Hearing aids, if applicable
• Current medication list, including dosages and prescribing clinicians

• Any applicable exemption, waiver, or Skill Performance Evaluation documentation
What the DOT Physical Includes
• Review of your health history and current medications
• Blood pressure, pulse, height, and weight
• Vision and color-recognition testing
• Hearing testing
• Required urinalysis for specific gravity, protein, blood, and sugar
• Physical examination of the body systems listed on Form MCSA-5875
• Medical qualification determination and certification when applicable
